INFORMATION AND INSTRUCTIONS FOR APPLICATION FOR EXEMPTION

1. The Application Form together with the Undertaking and Agreement must be filled in completely and signed.
   
   
2. Applicants are to submit the documents and information set out below :

   Copies of relevant Grant of Written Permission from Urban Redevelopment Authority (all pages) Business profile of entity (extracts from the Instant Information Service, Accounting & Corporate Regulatory Authority (ACRA) ) or documents of registration with the Registry of Societies Club's Constitution (if applicable) Any other information as requested by the Hotels Licensing Board.

3. Every application must be submitted to :

   Hotels Licensing Board
   c/o Singapore Tourism Board
   Tourism Court, Level 1, Reception Counter
   1 Orchard Spring Lane (off Cuscaden Road)
   Singapore 247729.

4. Where there is insufficient space under the items, separate sheets of paper may be used as attachments. Such attachments must be signed and dated by the applicant.
   
   
5. Please ensure that all information furnished is true and correct.
   
   
6. If there are any changes in the applicant's or organisation's particulars, the Hotels Licensing Board must be informed in writing immediately.
